2 years old, still no words — wait or get help?

This is a developmental concern that should not be ignored.

By 2 years old, many children can:

  • Say at least 20–50 words
  • Point to body parts
  • Follow simple instructions
  • Combine two words (“mama nak”, “nak susu”)

No single meaningful word at 18 months old and no combined phrases at 24 months old is a red flag

Possible causes include:

  • Hearing problem
  • Developmental language delay
  • Autism spectrum disorder
  • Global developmental delay
  • Less commonly, neurological conditions

“He will talk when he is ready, right?”
“Your father was a late talker too, and he eventually caught up.”

Some children do catch up, but we cannot predict reliably which child will catch up and which child needs early intervention.

What assessment is important?

  • Hearing test
  • Developmental assessment
  • Observation of social communication
  • Parent interview

Signs that increase concern of Autism Spectrum Disorder (ASD)

  • Poor eye contact
  • Does not point to show interest
  • Does not respond to name
  • Prefers to play alone
  • Rigidity, Restricted Interest, Repetitive Behaviour
  • Sensory Issue
