Started Preschool and Keeps Coughing - Normal or Lung Problem?

It is one of the most common worries among parents.

The short answer

For many children, frequent coughing during the first 6–12 months of preschool is common.

  • Healthy preschool children may get 6–12 viral infections per year.
  • Each cough can last 1–3 weeks.
  • As a result, some children may cough for 8–12 weeks in total over a year.

International guidelines from the American Academy of Pediatrics (AAP), NICE (UK), and Royal Children’s Hospital Melbourne recognize that repeated viral coughs are common when children first attend daycare or preschool.

When is it usually normal?

  • Cough mainly during a runny nose or flu
  • Child remains active
  • No breathing difficulty
  • Growing well
  • Cough improves/ resolves between infections

When should we look deeper?

Seek a medical assessment if your child has:

  • Cough for more than 4 weeks
  • Night cough every night
  • Wheezing or whistling sound
  • Shortness of breath
  • Poor weight gain
  • Recurrent pneumonia
  • Cough triggered by exercise, cold air, dust, or other allergens

What parents can do?

  • Ensure adequate sleep and balanced nutrition to support immunity.
  • Consider age-appropriate supplements when necessary to address nutritional gaps.
  • Keep vaccinations up to date, including annual influenza vaccination.
  • Encourage hand hygiene.
  • Avoid cigarette smoke exposure..
  • Avoid starting antibiotics without medical advice.

💡 Take-home message: A child who coughs often after starting preschool is not necessarily weak or having “lung damage.” The pattern of the cough is more important than the number of times the child gets sick.
